What Causes Facial Sagging?

The Superficial Musculoaponeurotic System (SMAS) is the fibrous layer of tissue that covers the deep structures of the face. In a surgical facelift, this is the layer surgeons physically pull. Over time, this “internal sling” weakens, leading to jowls and a softened neck.

Youthful faces have well-positioned fat pads in the cheeks. With age, these pads thin and shift downward due to gravity, accumulating at the jawline and creating “heaviness” in the lower face.
